Job Description
Duties include but are not limited to:
- Provide care for Veteran patients in a manner which is reflective of excellent clinical judgment, medical knowledge, clinical skills (medical interviewing, physical examination, and procedural skills), humanistic qualities and professionalism.
- Outpatient clinic setting in which the primary care physician sees 12-14 patients per day
- Personalized, patient-centered approach that takes into consideration the Veteran's goals, diagnoses, and physical, functional, and psychosocial needs.
- Initiate and facilitate treatment and transfer in emergency situations
- Manages an assigned panel of patients, and helps provide coverage for the needs of other Veterans enrolled for primary care at the clinic
- Refer patients as needed to appropriate specialists, health care facilities, and coordinate with home health services.
- Document assessments and plan of care for each patient in accordance with standards of care, policies, and procedures.
- Become proficient in and engage in Tele-Health to home visits (e.g., VA Video Connect)
- Assist with coordinating care received in the community from non-VA healthcare entities
- Maintain state licensure, PECOS enrollment, and knowledge of new and current clinical methods
- VA Patient-Aligned Care Team (Patient-Centered Medical Home) model with each team having a dedicated Registered Nurse Care Coordinator, Clinical Associate (e.g. LPN), and Administrative Associate (e.g. Clerk).
- The clinic also has a Clinical Pharmacy Practitioner, Social Worker, and Mental Health staff to support the care of enrolled Veterans.
Preferred Experience:
- At least 2 years of experience solely providing face-to-face general medical and ambulatory outpatient clinic-based primary care to adults
- Family Medicine or Internal Medicine Board Certification
- Expertise in preventative and primary care health services for women, including, but not limited to cervical cancer screening, breast cancer screening, and bone density screening.
Work Schedule: Monday - Friday (8:00am to 4:30pm)
